Sending Your Case

At Kelley Dental Laboratory, we want to make every aspect of work with us as easy and convenient as possible. We have created tools to allow you to print a UPS shipping label and schedule a UPS pickup at your practice.

Print a Shipping Label

To get started, please fill out the form below with all relevant information. The form will generate a unique and trackable shipping label. Please be sure to print a version of the shipping label for your records.

Please note: if you need to print multiple labels, please be sure to refresh the page after you print your first label.


Schedule a UPS Pickup

Once your package has a UPS shipping label attached, you can schedule a pickup. Please provide your information below and a UPS driver will arrive at your practice to pick up your case.

Please Note:
Pick up must be at least 3 hours later than at the time you are scheduling at.
You can not schedule a pick up for same day after 2:00 PM EST
